How to Change People … Whether They Want to or Not

By Jane Firbank How often have you wanted to change someones behaviour … and found yourself nagging, pleading, shouting, complaining or just plain giving up, because nothing you do seems to work? And if its a relationship youd like to improve … how often have you heard people say, oh, thats not possible unless you BOTH want to change? Well, tell that to the advertising industry … or your childs schoolteachers! Of course you can change peoples behaviour … whether they want to change or not, and whether they know it, or not. After all, every dealing you have with someone influences to some extent how they feel about you and how theyre likely to behave with you next time they see you. So, you may just as well bring the process under some conscious control … and heres how. The first rule for changing the way someone behaves is blindingly obvious — yet its easy to get so stressed you just dont see it. If whatever youre doing isnt working, then — however reasonable, sensible, justified it may be — stop doing it! Do something else. As they say, if you go on doing what you have been doing, then youll go on getting what you have been getting! Im reminded of Brenda. Her problem was her almost-teen daughter. The music practice she wouldnt do. The room she wouldnt tidy. The house resounded to furious slanging matches, endless nagging — all futile, except to make the daughter feel attacked and unloved, and the mother feel helpless, frustrated … and unloved. Stopping fighting, letting the room and the music take care of themselves, was the essential first step towards rebuilding a mother-daughter relationship which both could enjoy and value … and in which change could take place. Once youve stopped doing the wrong things, the right ways to create more productive, pleasant and life-enhancing relationships with the people in your life are really so simple. * Reward. First, find some effective rewards. Whether its a smile or a compliment, a pay rise, a night out, a hug or a chocolate, rewards are things which make the person feel good — not necessarily what you think they should want. The wrong reward could turn someone off. Say you praise someone in public, and that embarrasses them. Or your compliments a backhander, with blame for the fact they didnt do it before, and pointing out that they are now being good, under your control … we all know those compliments, and resent them! They are certainly not rewarding. Of course, you may ask, ‘Why on earth should I reward someone for just doing what they should?’ Forget shoulds and oughts. Its simple. You reward them because you want them to do it again. * Be rewarding most of the time, then some of the time, then just occasionally. If youre too obviously rewarding, every single time, people may well suspect they are being manipulated! Plus, as soon as you stop rewarding every time, they may rather quickly stop doing what you want. Imagine, your new husband brings you flowers every weekend without fail. Then, one week, he misses. Has he fallen out of love? Met another woman? Whereas, if he just sometimes, at unpredictable intervals, turned up with roses, youd feel great when he did but he could miss out for a long time before you began to feel unappreciated. * Having found some appropriate rewards, you apply them as quickly as possible whenever the behaviour moves, even a little bit, in the direction you want. You stay calm and pleasant but you dont give attention to what you dont want. One of my patients found her mothers almost daily telephone calls, guilt-provoking and destructively critical, left her really shaken. A simple strategy made a huge difference. When her mother said anything neutral or pleasant, Julie would engage in the conversation and sound interested. As her mother started on Julies faults, or tried to settle down to an hours moan … Julie sounded bored. If her mother went on regardless, Julie heard the doorbell or remembered an appointment after about a minute. If youve seen those programmes where parents are taught how to deal with screaming toddlers, youll recognise this as the Little Angels or Nanny 911 technique of behaviour shaping. Rewarding the good, ignoring the bad, and time out — ending the phone call — if the bad continues. It works just as well with adults. It gently moved Julies mother towards talking more appropriately to her daughter and also helped Julie feel in charge. Her buttons were no longer being uncontrollably pushed. Instead of being immersed in the situation, she gained a feeling of distance and control. And … just as those screaming toddlers look much happier once calm control is achieved … so Julies mother also benefited as, for the first time, they began to achieve some pleasant and mutually rewarding phone chats. So dont feel bad about using these techniques! * To stop behaviour you dont want, remember, if what someone does isnt noticed, appreciated, or rewarded, theyre likely to stop doing it. But if its the only way they have to get attention … then ignoring it will just make them up the stakes. There MUST be an alternative, acceptable way to get attention and reward, which, after all, we all need. Chloes husband went in for days of silent sulking. I suggested that she simply live her own life when he sulked, instead of frantically coaxing him to say what was wrong and trying desperately to please him and get him to respond to her. But withdrawing attention when he sulked was only half the answer — she also had to give more warmth, sharing, appreciation and fun when he wasnt sulking. Knowing you have powerful means for improving your relationships helps you feel calmer and more on top of things, less stressed so that your own natural warmth and personality can come out. What’s Up With Botox?

By Mike Jones BOTOX is part of a family of drugs called “botulinum toxins”. Specifically, it is the Registered Trademark and brand name for “botulinum toxin type A” which is manufactured by a company called “Allergan”. Nothing else that is made by any other company is allowed to be called BOTOX. Botox is actually a first cousin to the dreaded botulism food poisoning that is caused by the evil bacterium Clostridium botulinum. Yikes! That sounds scary enough to make you stop reading right now — but don’t run away quite yet. Just as atomic energy can be used for good and bad, there’s room in your life for Botox if used and administered properly. Although you may have only heard of it recently, Botox has been around since the early 1960’s when it was first approved for use as a treatment for Strabismus (crossed eyes). It later received approval from the U.S. Food and Drug Administration as a treatment for three specific medical conditions known as blepharospasm, strabismus, and cervical dystonia. Essentially, each of these conditions involve the appearance of involuntary muscle contractions. In order for a muscle to move, it must receive a signal from your brain. That signal is carried to the muscle by a chemical called acetylcholin. If your body produces too much acetylcholine then the affected muscles will go into “spasm” or begin twitching. BOTOX blocks the release of acetylcholine to the affected muscle which stops or reduces the spasm. Since Botox is injected only into the affected muscle the rest of the body is not affected. Unless you have one of the diseases that regular Botox is designed to treat, the chances are your real interest is in a derivative called “BOTOX Cosmetic”. Although most people simply refer to this derivative as Botox, there is a marked difference. BOTOX Cosmetic is a protein which is produced by the Clostridium botulinum bacterium. It is FDA approved for reducing the activity of the muscles that cause “frown lines”. According to the manufacturer, “Treatment with BOTOX Cosmetic can visibly smooth and soften moderate to severe frown lines between your brows. An improvement can be seen within days and may last up to 4 months, although results may vary. In clinical trials, nearly 90% of men and women surveyed rated the improvement in the appearance of frown lines between their brows as moderate to better 1 month after treatment.” This means that while BOTOX Cosmetic doesn’t work for everyone, there is a 90% chance that it will work for you. BOTOX Cosmetic is administered by injection directly into the muscles that are causing your frown lines. The process takes 10 minutes or less and there is no “recovery period”. That makes it perfect to get done during your lunch break or anytime that you have a free 10 minutes. A “normal” treatment can last for up to 4 months although this time period varies widely among different people. Regardless, the manufacturer recommends that you get treatments no more frequently than every 4 months. Here are some guidelines for you to consider: BOTOX Cosmetic may not be for you if you have any diseases that affect your nerves or muscles or if you are pregnant, planning to become pregnant soon, or are nursing. Be sure to give your doctor a COMPLETE listing of all medications that you are taking prior to receiving a BOTOX Cosmetic treatment. Once you stop treatment your frown lines will gradually reappear until they look the same as before you began the BOTOX Cosmetic program. It is not a permanent cure for your wrinkles! Emergency Medical Services � How to Become an Emergency Medical Technician

By David Joiner Emergency Medical Technician (EMT) a qualified, licensed health care professional who generally provides pre-hospital emergency medical care to the ill or injured patient. Working in the field of Emergency Medical Services (EMS) can be a very rewarding and challenging experience for those individuals who are committed to the safety and well being of their family, friends, neighbors, and more often complete strangers in the communities they serve. General Pre-qualifications for most States: Applicant to an Accredited EMT Program must be at least 18 years old. The applicant must possess a High School Diploma or Equivalent. Some EMT programs may require a placement exam, i.e. colleges that require an entrance exam for all students. EMT Programs in most states consist of approximately 110 hours of classroom and practical training and at least 24 hours of hands on clinical experience, usually at a local hospital and EMS Provider. Classes can be completed in as little as 14 days, although most programs meet once or twice a week for 3 to 4 hours over a 5 or 6 month period. During the classroom and practical training sessions of the EMT program a variety of topics concerning pre-hospital patient care will be discussed. These topics are generally broken down into six to eight modules as Ive listed below. Please keep in mind that some programs may differ slightly in the way or order the information is presented, but you can at least get a very good idea of what you will learn and need to accomplish to successfully complete the EMT program you choose to attend. Module I: This module is generally considered the preparatory portion of the EMT program in which the EMT student learns and participates in the discussion of such subjects as, Introduction to Emergency Medical Care, Protection Methods for the EMT, Legal and Ethical Issues, General Patient Anatomy and Physiology, and Lifting and Moving Patients. Module II: The Patient Airway Management and Cardiopulmonary Resuscitation (CPR) module. In this portion of the course you will learn in addition to CPR, skills such as additional airway management techniques, using airway adjuncts, and oxygen administration. Module III: The Patient Assessment Module is perhaps the most challenging part of the EMT Program consisting of, sizing up the medical or trauma scene, the initial or primary patient assessment of the medical and trauma patient, how to obtain a pertinent patient history, obtaining vital signs, the ongoing or secondary patient assessment, communicating your findings and proper patient documentation. Keep in mind that although we speak of the patient in general terms we are covering various age groups, including, pediatrics (newborns, infants, and toddlers, and children), adolescents (teenagers), adults, and geriatric (elderly) patients. Module IV: This particular module usually covers medical emergencies. The EMT student not only learns to differentiate between several types of medical emergencies, but performs the recommended treatment involved to assist the patient before and during transportation of the patient to a medical facility. Some of the topics in this module include, Respiratory and Cardiac Emergencies, Altered Mental Status, Diabetes, Allergic Reactions, Behavioral Emergencies, Poisoning and Overdose, Environmental Emergencies, Obstetrics and Gynecological Emergencies. Module V: Traumatic Emergencies Module is another especially challenging part of the EMT program. In this area of study to will learn how to recognize the different types of shock, how to control bleeding, soft tissue injuries, injuries of the head, neck, and spine, and injuries to the musculoskeletal system. Module VI: Medical and Traumatic Injuries to Infants and Children Module VII: A variety of skills are learned in this module including, gaining access to patients, certain types of rescue operations, topics covering ambulance operations and special situations. While this is a general overview of a typical EMT program, some programs may cover or require additional involvement by the EMT student including advanced airway management, dealing with stress, assisting the advanced paramedic provider and more. After completing the didactic and skills portions of the EMT program usually with a minimum of a 70% grade average, the perspective EMT must also in most states take and pass the National Registry practical and written exams. After successful completion the student can then apply to be licensed in the state they reside. While most states require an EMT to be Nationally Registered, the EMT usually must challenge the EMT test of another state to be able to practice in that particular state. Some EMTs have licenses to practice in more than one state. This usually occurs when an EMT lives close to two state lines and has employment in both states. In conclusion as I mentioned earlier, being an EMT can be both rewarding and challenging, but it is NOT easy. It takes a unique type of individual committed to helping those who are ill or injured and most EMTs seem to develop the required traits for success in the EMS Industry. African Hoodia Cactus

Xshoba or xhooba or African hoodia cactus was used by the San Bushmen to treat indigestion, minor skin infections and as an appetite and thirst suppressant during long hunts. Tummy Tuck after Pregnancy

By Tara Pingle Giving birth to a baby is one of lifes supreme joys. But after its all over, and we start returning to our former selves, sometimes we arent 100% pleased with our new shape. That pouch of extra fat and skin below the belly button sometimes wont flatten out again, especially after the second or third baby. The muscles, fascia, and skin have been stretched to accommodate the baby, and even though breastfeeding helps to pull them back into shape, and proper diet and regular exercise continue to help, sometimes they remain stretched-out. If the belly hasnt reverted to its pre-pregnancy flatness by the end of 6 months, it probably will never do so without help. So perhaps we start to wonder about getting a tummy tuck. Further Pregnancies If you think you may get pregnant again, postpone the tummy tuck until thats all over. A pregnancy after the tummy tuck could undo all the work, wasting your money and putting you back where you started. Tummy tucks and the structures of the abdomen Right below the skin is the stuff we dont want: the fatty tissue. Below that is the fascia. Fascia means a sheet of tough connective tissue, mostly collagen (which is a protein we have in bones, muscle, skin, and throughout our bodies). This particular fascia is our natural girdle. Its a sort of fibrous fabric that extends from the ribs down to the pubic bone, across the whole width of the abdomen. When its not stretched out of shape, it keeps our tummy flat, with the cooperation of well-toned muscles. Below the fascia are the muscles. In a tummy tuck, these are not exposed and not worked on. We might read that a tummy tuck will tighten the muscles, but really it tightens the fascia which lies over the muscles. What causes the belly bulge is this stretched fascia, which isnt as resilient or elastic as muscle tissue. What does a tummy tuck do? Briefly, a tummy tuck (abdominoplasty) removes extra fat and skin from the abdomen and tightens the abdominal fascia. This flattens the tummy and sometimes narrows the waistline as well, since excess tissue is removed. Many times, the belly button is moved to a new location to match the new contours. Tummy tucks are usually done in a hospital or surgical center, with general anesthesia, although your stay will be just one overnight. They take anything from 1 to 4 hours, depending on how much tissue is to be removed. When he sutures the incision closed, your cosmetic surgeon will place a drain under the skin, or perhaps two. They prevent fluid from pooling under the skin (seromas), by removing the excess fluid. Therell be a scar, but below the bikini or underwear line. Mini tummy tucks A mini tummy tuck does the same things, but focuses only on the lower abdomen, below the belly button. A full tummy tuck procedure works on the entire abdomen, up to the ribs. Mini tummy tucks are done on an outpatient basis, usually with local anesthetic and some IV sedation. They leave a smaller scar, low on the abdomen as with a full tummy tuck. Combining the tummy tuck with other procedures Tubal ligation If youre very clear in your mind that you want no more pregnancies, your tummy tuck can be done at the same time as a tubal ligation (tying your tubes). Hysterectomy If your doctor advises you that you need a hysterectomy, it can be done along with the tummy tuck. Liposuction Since fatty tissue is removed in a tummy tuck, the procedure can be done with liposuction. What a tummy tuck will not do It wont remove stretch marks from your abdominal skin, but simply move them lower down. If the layer of fat in your upper abdomen is very thick, a tummy tuck wont remove it all. Sometimes liposuction can remove some, but too much liposuction of this upper area, done at the same time as the tummy tuck, can affect blood circulation and cause complications. Recovering from a tummy tuck At first therell be pain, swelling, and numbness, but not bruising. The plastic surgeon will have applied bandages and inserted drains. Pain: Your surgeon will of course prescribe a pain medication and youll probably take that for a week or two. Swelling: The swelling should subside within 5 or 6 weeks, perhaps as soon as 2 weeks. Numbness: The numbness may last several months but is normally not permanent. Drains will be removed when drainage eases up, after a couple of days, or maybe after a week or two. Avoid taking any showers until theyre removed; just take sponge baths. Bandages are removed after 3 or 4 days. Youll feel like resting, but dont rest all the time. To encourage good blood circulation, you should walk around every hour or so and perhaps do light household chores. But avoid anything that feels uncomfortable. Wiggle your feet periodically and keep your hips gently flexed forward, to avoid putting tension on the stitches. You can take baths after about two weeks. Therell probably be surgical tape over the stitches and you should leave it on until it comes off by itself, or until your surgeon removes it. In the meantime, keep it dry. If your work isnt physically active or strenuous, you can be back after a week or 10 days. Otherwise, its wise to wait for a month or 6 weeks. You can drive again after you stop taking the pain medication. After a month, you can be back at the gym or jogging etc. The final result wont be fully evident until the scar heals, in about a years time. Finding a good cosmetic surgeon Since cosmetic surgery has become so popular in the last 10 years or so, some doctors have taken short cut training to take advantage of this at a disservice to the general public. You want a properly trained and experienced plastic surgeon, not one who might gain his basic experience on you! Choose a plastic surgeon whos certified by the American Board of Plastic Surgery (ABPS), and one whos a member of the American Society of Plastic Surgeons (ASPS) and the American Society for Aesthetic Plastic Surgery (ASAPS). While you should by all means listen to your friends and acquaintances who may have had cosmetic surgery and want to recommend their doctor to you you should do your homework too, and check the doctors credentials. Some questions to ask when interviewing plastic surgeons Do you have hospital privileges, and if so, which hospitals? Can you do the procedure in the hospital if I want to? If you do surgery in other facilities, are they accredited? How many breast reductions have you done? Can I speak to some of your previous patients about their experience with it?
